Primary Care / Medical Home

Children's Medical Services (CMS) believes that every child should have a primary care doctor who serves as that child's medical home. Children with special health care needs specifically need a primary care doctor. Not only is it important to get specialty care services for their special condition, but children with special health care needs should also receive routine check-ups and childhood immunizations from a primary care doctor.

At the primary care doctor's office, the doctor and a nurse work together with parents to develop a care plan and assist the family in getting services your child needs. Brothers and sisters of children enrolled in the CMS Network are also eligible for services in the CMS Primary Care program.

The following services are provided through the Primary Care program:

  • Office visits for sick and well child care
  • Immunizations
  • Diagnostic tests such as, labs, x-rays services, etc.
  • Prescriptions for medicines and specialty equipment
  • Referral to specialists and other health care providers
  • Care coordination to maximize community resources and provide support to families.
  • Twenty-four hour / seven-day-a-week call coverage
