Florida Department of Health in Volusia County Child Immunizations

Vaccinating on time means healthier children, families and communities.

The Florida Department of Health offers no-cost vaccines for eligible children. A small administration fee may apply.

Vaccines are the best defense we have against serious, preventable, and sometimes deadly contagious diseases. Immunizations reduce absences both at daycare, school and other activities and decrease the spread of illness at home and in the community.

Immunizations for children are always free at the Florida Department of Health in Volusia County. Prior to enrollment into a childcare facility, or to attend school (kindergarten through 12th grade) children are required to have a Florida Certificate of Immunization (DH 680 form, a.k.a. Students entering 7th Grade
Before students enter the 7th grade, proof of current vaccinations are required. The vaccines listed below protect adolescents from very serious preventable diseases:

To assist parents with getting ready to go back to school, the Florida Department of Health participates in numerous back-to-school events throughout Volusia County.  

Need an immunization appointment?
Appointments are required for child immunizations. Contact our Appointment Line (386) 274-0509, to make, change or cancel appointment for your child. Customer service representatives are available to assist clients 8 a.m. to 5 p.m.

Vaccination Records
Transferring students in need of an official immunization record (DOH Form 680) must provide their immunization record from their previous provider. Records may be submitted in advance by:

  • Faxing to: DOH-Volusia Medical Records, (386) 274-0518.
  • Mailing to: The Florida Department of Health in Volusia County, Attn: Medical Records, Bin 103, P.O. Box 9190, Daytona Beach, FL 32117
  • Dropping it off to one of the Florida Department of Health locations in Volusia County.
