My GULF Care

My GULF Care is a collaborative community resource between Sacred Heart Hospital on the Gulf and the Gulf County Health Department. The mission of My GULF Care is to connect members of the community with resources that will improve their health.

My GULF Care can assist clients with the following services:
  • Primary Care Physicians
  • Diabetes self-management programs
  • High blood pressure self-management programs
  • High cholesterol/lipid self-management programs
  • Dental Services (Medicaid and limited uninsured)
  • Medication Assistance Programs
  • Health Coaching Program
  • Food Assistance
  • Clothing Assistance
  • Counseling
  • Financial Assistance
Care Management Program Description
Our Care Manager will provide you with the tools you need to better control your chronic conditions. Care Management is provided for clients who have Diabetes, Hypertension, and Hyperlipidemia. One-to-one classes are designed around your needs and in a setting that is most convenient for you. If you have trouble controlling your blood sugar, blood pressure or cholesterol, Care Management may be able to help you.

Health Coaching Program Description
As part of a comprehensive community resource initiative, MyGULFCare offers a Health Coaching Program. Our health coaching program offers clients assistance in lifestyle changes, such as weight management, stress reduction, and fitness plans. The Health Coaching Program involves one-to-one K.N.E.W. (Knowledge, Nutrition, Exercise, Wellness) Plans, as well as optional group and community activities.

Eligibility Requirements for My GULF Care Health Coaching and Care Management Programs
  • US citizen or legal resident
  • Gulf County resident
  • Low household income
  • Have Medicaid insurance or are uninsured
For Care Management Program:
You have a regular family doctor but need instruction to help you control your diabetes, high blood pressure or high cholesterol.

For Health Coaching Program:
You have a regular family doctor that you have seen within the past two years and are interested in making healthy changes in your life including, but not limited to: Weight loss, increased physical activity, stress management, healthy eating or smoking cessation.
