State and Community Health Assessments

Comprehensive community health assessment and health improvement planning are the foundations for improving and promoting healthier Florida communities.  Our 67 counties use a commong process for collecting, analyzing and using data to educate and mobilize communities, develop priorities, gather resources, and plan and implement actions to improve public health.   

At the state and local levels, four critical assessments provide the basis for action:  community health status assessment, forces of change assessment, local public health system assessment using the National Public Health Performance Standards Program (NPHPSP), and assessment of community themes and strengths. Assessment findings inform the selection of strategic community health priorities.  Goals and strategies and measurable objectives are used to develop a community health improvement plan that includes implementation strategies and action plans.   Two important tangible products of thes efforts are state and community health status profile reports and state and community health improvement plans. The parallel processes result in state and local documents reflecting each area's needs and priorities.

Florida's State Health Assessment

Florida and its health partners have conducted a comprehensive health assessment.   Culminating in the State Health Assessment, a comprehensive perspective of Florida's health status, resources, strengths and opportunities for improvement is presented.   Individually, each assessment yields an in-depth analysis of factors and forces that impact population health. The four assessments taken together contribute to a comprehensive view of health and quality of life in Florida and constitute Florida's State Health Assessment report.  Because it provides essential information for discussion, prioritization and action, a state health assessment process is the foundation for improving and promoting the health of Florida's residents.

Florida's State Health Improvement Plan

Florida's State Health Improvement Plan (SHIP)  is a statewide plan for public health partners and stakeholders to improve the health of Floridians.  With goals, strategies and objectives that cross agencies, the plan identifies priorities specific to Florida’s needs, raises awareness of public health issues, provides a common agenda for health, links statewide efforts to national initiatives, increases accountability and builds collaborations that produce results.
