In this demonstration, we will go over what you need to do when you are unable to submit events for a particular reporting month. All key user functions, can be found under the utilities tab. If you do not have the proper permissions, this tab will not be visible to you. Click on the 'Utilities' tab to start this demonstration.... Sometimes, due to various local problems, you might be unable to submit you local records. This can occur if there is a network failure, for examle. If you are not currently late with a submission, you may apply for an extension, to give you the time to fix local issues. If approved, you will not be considered late, as long as your submission is uploaded by the approved extension date. Click on the 'Apply For An Extension' link to continue... This is the application screen. Here you must fill out (most is auto-populated) all of your contact information, the extesion (reporting) month, and the date you estimate that you will be able to submit the records. Also select a reason, and any comments you feel will help to explain your issue. Select the 'Personnel' list item for an Extension Reason to continue... Once you have completed all information, click on the submit button. You request will be submitted to the Data Unit at the Florida Department of Health. Click on the 'Submit' button to continue... Once submitted, you will recieve notification at the bottom of your screen. This concludes this demonstration. Click anywhere (in this demonstration screen) to finish...
