Vaccinations for Kindergarten Through 12th Grade

Avoid Long Lines and Crowded Waiting Rooms

Vaccinations for children (under 18) are offered at no cost to you! The Florida Vaccines for Children (VFC) Program is federally administered and funded by the Centers for Disease Control and Prevention (CDC) through the National Center for Immunization and Respiratory Diseases to provide vaccines to children at no cost to the parent/guardian.

Kindergarten Through 12th Grade Vaccines (children entering, attending, or transferring to Florida schools):

  • Four or five doses of diphtheria, tetanus, and pertussis vaccine
  • Three or four doses of polio vaccine
  • Two doses of measles, mumps, and rubella vaccine
  • Two or three doses of hepatitis B vaccine
  • One dose of varicella vaccine* (kindergarten effective school year 2001-2002, then each year an additional grade)
  • Two doses of varicella vaccine* (kindergarten effective school year 2008-2009, then each year an additional grade)

 

*Varicella vaccine is not required if there is a history of varicella disease documented by the healthcare provider.

11-12 year-old vaccines:

  • Tetanus-diphtheria-acellular pertussis vaccine (Tdap)
  • Meningococcal vaccine (MCV4)
  • Human papillomavirus (HPV) vaccine

If this is your first visit for immunizations, bring child's social security card and past shot records.
