Who is Eligible?
Children birth to age 20 receiving Medicaid
Patients over the age of 20 receiving Medicaid may qualify for emergency dental care.
Emergency dental care is only provided during normal hours of operation.

The Dental Clinic is open from
7:00 a.m. to 5:30 p.m., Monday through Thursday
is closed Friday

How do I get an appointment?
Call (850) 526-2412 x129

You must bring the following to your childís appointment:
your childís Social Security Card and Medicaid Card;
your Driverís License or other form of photo ID.
If you are not the parent, you will need to bring proof Guardianship or custody.

Broken Appointment Policy:
When appointments are made they are for your convenience and we expect them to be kept. If you fail to
keep an appointment or show up late, you are depriving someone else of this time. It is your responsibility
to keep track of your childís appointment. If you are unable to keep an appointment, please notify us at
least 24 hours in advance. Failure to do so may result in suspension or dismissal from the dental program.

                    Mobile Dental Program

Jackson County Health Department Dental Program provides a school based dental care program, Smiles On Wheels.
The mobile dental unit visits elementary schools in Jackson and Holmes Counties providing services to Pre-K through 5th Grade students who are eligible for Medicaid Managed Insurances.  The services provided to these students are dental examinations at least once a year, dental cleanings, fluoride treatments, oral hygiene instructions, and sealants as needed.  Some emergency treatment may also be done to get a student out of immediate pain.  The mobile unit is staffed by a licensed dentist and two dental hygienists.
