Immunizations

Immunizations are provided free for eligible children ages 6 weeks - 18 years.

Adults (age 19 years and up) will be charged a fee.

Flu Shots

The Florida Department of Health in Santa Rosa County is now offering flu vaccines for adults and children. Please call the Health Department (850-983-5200) for pricing. Cash, credit cards, and checks are accepted. We also accept Medicare Part B for flu shots if it is your primary insurance. Vaccines are free for eligible children 18 years old and under through the Vaccines for Children program. Please call for more information.

The Immunization Guidelines (.pdf 54kb) for the State of Florida cover entry, attendance or transfer to preschools, public/non-public schools (K-12), licensed daycare facilities, and family day care homes. The school the child attends will have on file a Florida Certification of Immunization (also referred to as a "Form 680").

Hours of Operation:

Immunizations are provided on a walk-in basis, no appointment is necessary.

Due to severe damage caused by flooding in April, the Florida Department of Health in Santa Rosa County Midway location remains closed until further notice. Immunization Clinic hours at the health department's main facility, 5527 Stewart Street in Milton, will be temporarily expanded from June 16 through August 14. Expanding clinic hours is an effort to provide additional opportunities for residents of the Midway/Gulf Breeze area to access immunization services.
