From June 2010 through June 2011, the St. Johns County Health Leadership Council completed a comprehensive, county-wide Community Health Assessment using a nationally recognized approach, called MAPP (Mobilizing for Action through Planning and Partnerships). The Council then used the data collected in the County Health Assessment to develop a Community Health Improvement Plan.

Championed by the St. Johns County Health Department, the St. Johns County Health Leadership Council is comprised of a diverse group of health, governmental, social service, business, and educational leaders that make up the local Public Health system. The Health Leadership Council’s mission is to promote, protect and improve the health of all people in St. Johns County.

Although St. Johns County remains one of the three healthiest counties in Florida, according to the Robert Woods Johnson/University of Wisconsin’s Population Health Institute’s 2011 County Health Rankings Report, the 2011 Community Health Assessment identified a number of strategic health objectives that are opportunities for improvement in St. Johns County. Following analysis of the community feedback and statistical health data that was collected for the Assessment, the Health Leadership Council developed an overarching list of community health objectives where they could have the greatest impact. These objectives include:

  • Reduce the percentage of low birth weight infants
  • Reduce the morbidity and mortality of leading types of cancer (i.e. lung, breast and colon)
  • Reduce substance abuse
  • Increase access to dental care
  • Increase access to mental health care
  • Develop a community health/social services resource tool
  • Improve Public Health system collaboration

The Council has identified measures and performance targets for these strategic objectives, and will implement action plans to address them, over the next three year period.
