What is Social Services?All clients that would like to be seen at the Florida Department of Health in Clay County are directed to come in for a social services assessment. This allows our staff to determine eligibility for clinic services, qualify clients for our sliding fee scale and make referrals to community services and programs that they may not be aware of.
What do I need to bring?Please review this document to be prepared when you visit one of our counselors.
When can I come in?Social service assessments are done on a walk-in basis, Monday through Friday, between 8:00 and 10:30am, or between 1:00 and 2:30pm.
Other resources and additional programs managed by Social ServicesCounselors are available to assist with the following applications:
Florida Breast and Cervical Cancer Early Detection Program: Grant program for uninsured women 50 to 64 that qualify financially to pay for PAP and mammograms
Susan Komen: Grant program for uninsured women under age 50 to pay for screening and diagnostic mammograms
WeCare: Provides specialty referrals for clients at or below 150% of the federal poverty level that have no other resources
Diabetic Supplies: Provided at a reduced rate to clinic patients
