Services
The Collier County Health Department serves as the county's vital records office for birth and death certificates. The Health Department can issue birth certificates for the years 1930 to the present for births within the State of Florida.

Birth certificates are available from the Collier County Health Department for ALL Florida counties. Birth Certificates are needed for:

A Passport
Admission to public school
To obtain a driver's license
A work permit
The right to vote
Eligibility for retirement
Social Security benefits
To prove citizenship
To obtain public assistance for dependent children

Eligibilty Requirements
Birth certificates can be issued to:
Parents
An individual 18 years of age or older
To legal guardians with proof of legal custody with proper identification

Types of identification accepted are: State I.D. Card, Driver's License, Passport, or Military I.D.
Fees or Payment
Birth certificates issued by the Health Department cost $10.00 for all Florida Births. A second certificate costs $6.00 if issued at the same time as the first certificate. We accept cash, check, money order, or Visa and MasterCard towards payment.
Enrollment Forms
On-line applications are not currently being accepted. Please download and print out an application for Florida Birth Record and mail it in with payment to the Post Office box above. Note: Please submit a copy of your ID if you are 18 or older along with your application and payment. If you are not 18 or older, submit a copy of your parent/guardian's ID.

Death certificates for other years must be requested from Jacksonville.

Eligibilty Requirements
Death certificates, with cause of death, can be issued to immediate family members of the deceased or to a licensed funeral director.

Cause of death is confidential information and can only be issued to immediate family members, which FS 382.025 defines as "spouse or parent, or to the registrant's child, grandchild, or sibling, if of legal age, or to any family member who provides a will, insurance policy, or other document that demonstrates the family member's interest in the estate, or to any person who provides documentation that he or she is acting on behalf of them." Cause of death cannot be shown for anything that will be filed as a matter of public record, i.e., real estate transfers; automobile transfers, recording of wills, etc.
Fees or Payment
Death certificates issued by the Health Department cost $8.00 each. We accept cash, check, money order, or Visa and MasterCard towards payment.

Please download and print out an application for Coliler County Death Record and mail with payment of $8.00 for each certificate, as well as a self-addressed, stamped envelope (legal-sized if ordering more than 5 certified copies), and mail to the post office box above.
