Protocol for Breaches of Confidentiality of CAREWare Data
This protocol outlines the steps that will be taken when there is a breach of protected health information entered into CAREWare. The protocol is intended to supplement DOHP 50-10-10 Information and Security Policy or local policies written to conform to the security requirements of Department of Health HIV/AIDS patient care contracts and subcontracts. More restrictive state or federal rules, regulations or laws take precedent over this protocol.
Confidential CAREWare Client Identifiers - Appendix A
This document details what identifiers are considered confidential for the purposes of the sharing of CAREWare data. You cannot share any of these identifiers electronically unless the electronic transmission is encrypted. This list is not exhaustive; please contact the Help Desk at 850-922-7599 and ask for a member of the CAREWare Team to return your call if you have any questions. If you must make a change to a client's record, inform the Help Desk operator that you need client data changed. Do not give any of the fields identified in the document to the Help Desk operators when calling in a ticket.
