Information or documents regarding a specific final order can be requested by visiting our public records webpage for information. Public record requests are not required to be in writing and you do not have to give your name or explain why you are making a request. Be sure to include the case number or final order number in your request to speed the research process.

To determine the discipline history of a particular licensed health care practitioner, you may use the online Licensee Lookup System, which provides a simple "Yes" or "No" under the topic "Discipline on File" and the current status of the license. In addition, you can view disciplinary action taken after June 2006, use our Final Order and Emergency Action search engine.

* Note: Emergency suspension or restriction orders are not considered final agency action, but are imposed as specified by s. 456.074, Florida Statutes, for serious violations relating to the commission of crimes, standard of care, or drug use, as well as for student loan defaults. The subject is entitled to a hearing before final action is taken by a regulatory board or by the Department.
