Current licenses expire at midnight, Eastern Time, on the last day of the birth month of the licensee.

Due to impacts from Hurricane Irma, Governor Rick Scott issued a state of emergency in every county in the State of Florida. Please see Executive Order 17-235, Emergency Order DEM 17-0002 and Emergency Order DEM 17-0003 for more information.

The Florida Department of Health, in accordance with Executive Order 17‐235 has therefore extended license expiration deadlines for pharmacists and radiologic technologist licensees whose license expires at midnight on September 30, 2017. The new licensure expiration date is midnight on October 30, 2017. Unless another extension is granted, any active license not renewed before this time will go into a delinquent status. Please continue to check for updated licensure expiration date information.

To ensure you receive your renewal notification from the department, your current mailing address must be on file. Failure to renew an active license before the expiration date will result in the license being placed in an expired status. Failure by an expired licensee to reactivate within ten (10) years of becoming expired renders the license null and void without any further action by the board or the department.

Licensees requesting to reactivate their license from inactive or retired status are required to pay additional fees and comply with specific continuing education requirements. Please contact the board office at mqa.rad-tech@flhealth.gov to request your reactivation requirements.

The department will renew your license upon receipt of:

  • Completed renewal application
  • Required fees
  • Continuing Education
