The primary function of the General Program is case management and care coordination.

The secondary function is to oversee the funded rehabilitatio and accessibility services that include things like transitional living, durable medical equipment, assistive devices, home and vehicle modifications and others.

1. What states are covered by this program?

  • You must be a legal resident of Florida to be accepted into this program.

2. How do you get started in the program?

  • The hospital will refer the patient to the Central Registry. Once in the Central Registry, a local case manager will be assigned to your case.
