If you want our statisticians to conduct analysis for you and provide you with the results, please send your request to:

In your request, please explain precisely which question(s) or variable(s) are of interest and how to analyze the data (e.g. data year, age or racial/ethnic group, region, etc.).  All requests will be processed in a timely manner on a first-come, first-serve basis. Once your study is finished, please provide a courtesy copy of any articles and/or reports to the Department of Health, Bureau of Epidemiology based on the MSHBS data provided from the request.

Please note: Florida has a very broad public records law.  Most written communications to or from state officials regarding state business are public records available to the public and media upon request.  Your e-mail communications may therefore be subject to public disclosure.

Dataset Request
If you would like to conduct data analysis, datasets are available in SAS, SPSS, or Excel format.  To submit a data request, please fill out the data sharing agreement form located at the link below and fax to 850-922-9299, to the attention of the Chronic Disease Section Administrator.
