COVID-19 Vaccine Information 

Thank you for your interest in serving your community by providing COVID-19 vaccinations.

Note: To become a COVID-19 vaccine provider your organization must be enrolled in Florida SHOTS and the Vaccines for Children/Adult Program.

COVID-19 Vaccine Administation for Health Care Providers

If you are a health care provider interested in administering COVID-19 vaccines, you will need to enroll as a COVID-19 vaccine provider. The first step in enrolling as a COVID-19 vaccine provider is to apply for a Florida SHOTS account if you don’t already have one. See the handouts below for more information on how to enroll as a COVID-19 vaccine provider.
