Community Integrated Mobile Health Services

The Community Integrated Mobile Health Services (CIMHS) is comprised of several programs such as the Mobile Health Unit, Florida Asthma Home Visiting Program, Fall Prevention and the Diabetes, Empowerment, Education Program (DEEP).

The Community Integrated Mobile Health Services (CIMHS) was established in December 2019 and comprises community outreach, home visit and post discharge follow up services to reach those who may have challenge with access to care.

CIMHS is very versatile as it adapts to the ever-changing physical environment where patients are engaged, such as the patient’s home where we are mindful to aim at mitigating social determinants of health. Our comfort zone is the interface of the healthcare sector, patients care team, and patients’ home. CIMHS utilizes a robust community resource network to ensure that gaps in the patient’s care are filled while they seek to live a quality life managing chronic illnesses.
