Companies and persons who wish to become providers of approved continuing education courses for the RT profession must meet and follow the requirements of Rule 64E-3.009, FAC, which includes submitting a completed DH 374- CE Provider Inforamtion Sheet and attachments to the department at RadCE@FLHealth.gov.
