Health Disparity Data
The Office of Minority Health will analyze and display secondary data and make recommendations to address data gaps. Health data are stored in the Florida Community Health Assessment Resource Tool Set (Florida CHARTS), Florida Health’s database for tracking the health status of the state.

Florida CHARTS
Florida CHARTS' Ask a Question feature allows you to receive feedback from the Florida CHARTS team regarding the data and/or information found on Florida CHARTS.
With Florida CHARTS, you can track the health status of your community. Whether it’s looking at your county’s AIDS case rate or identifying the number of births in your local census tracts, Florida CHARTS has the data you need. This data is displayed in profile reports, maps, data tables, and flexible query systems.
Community Tools
Florida CHARTS Life Expectancy Map provides theoretical estimates of the average number of years from birth a person is expected to live. It is based on current death rates by age group.Persons moving into or out of a geographic area, getting older and changes in death rates may change this estimate.This map can be accessed via Life Expectancy Profile on Florida CHARTS.
Florida CHARTS Community Map data is provided by the Florida Department of Health Bureau of Vital Statistics and the 2015 American Community Survey 5-year estimates. It includes census tract map, zip code map, Florida map, and county demographics. It can be accessed via Charts Mapper.
Florida CHARTS Social and Economic Factors Map utilize data from U.S. Census Bureau, American Community Survey 5-year estimates. It looks at county level community and social economic factors from 2016 to 2020. It can be accessed via Community Social and Economic Factors Report.

Cardiovascular DiseaseIn Florida CHARTS, health data on cardiovascular disease can be asssessed via chronic disease as a health indicator. This health indicator include diseases such as cardiovascular disease, diabetes, cancer, etc.
Diabetes
In Florida CHARTS, health data on cardiovascular disease can be asssessed via chronic disease as a health indicator. This health indicator include diseases such as cardiovascular disease, diabetes, cancer, etc.
Example reports:
The report for “Adults who have ever been told they had diabetes.”
The report for “Adults who are obese.”
Both the “adults who have ever been told they had diabetes” and “adults who are obese” can be viewed as county and state level data reports.
Maternal and Infant Mortality
In Florida CHARTS, health data on maternal and infant mortality can be assessed via the Maternal and Child Health health as an indicator. The maternal and child health indicator include statistics such as maternal deaths, infant deaths, fertility rates, etc.
In Florida CHARTS, vital statistics reports provides annual and provisional reports summarize data compiled from the original birth, death and fetal death filed with the Florida Bureau of Vital Statistics.
