Data Inquiries and Requests 

The Florida Birth Defects Registry is the only data source in Florida where demographic, clinical, maternal and child health information is combined for analysis and reporting on the occurrence of birth defects. We understand the importance of developing evidence based, data driven policies and research initiatives.

If you are interested in using FBDR data please email completed 1) FBDR Data Use Agreement form and 2)  FBDR@FLHealth.gov with approved copies of both the:  a) Vital Records Data Use Agreement, and the, b) Agency for Health Care Administration Data Use Agreement. As necessary, a where necessary, the DOH Institutional Review Board (IRB) application may also need to be provided.

Under Florida law, e-mail addresses are public records. If you do not want your e-mail address released in response to a public records request, do not send electronic mail to this entity. Instead contact these offices by phone or in writing.
