Insulin Distribution Program

The Insulin Distribution Program is a safety-net program and is only to be used when there is no other resource available for the insulin.

 

Central Pharmacy and County Health Departments (CHDs) with a licensed pharmacy are authorized to fill client-specific insulin prescriptions. CHDs without a licensed pharmacy must forward insulin prescriptions to the Central Pharmacy.

 

ELIGIBILITY DETERMINATION AND DOCUMENTATION

A person applying to participate in the program must complete the Insulin Distribution Program application and provide a current prescription for insulin. The application is available in English, Spanish, and Haitian-Creole.

 

Upon receipt of a completed application and prescription for insulin, the CHD will determine the applicant’s eligibility based on the criteria below:

  • Self-declared resident of Florida
  • Has diabetes
  • Uninsured, lacking insurance that covers insulin, or has an insurance deductible or copay that the applicant cannot afford
  • Has a net family income at or below 100% of the poverty guidelines
  • Has no more than $2,500 in private funds, bank accounts, or assets other than a homestead
  • Not a current Medicaid recipient

 

The CHD has the authority to make the final determinations of eligibility for the Insulin Distribution Program.
