Helping Hearts Community Health Integration (CHI) Referrals and Services

In partnership with the Florida Community Health Worker Coalition (FCHWC), the Helping Hearts CHI Referrals and Services is an initiative to establish and sustain a statewide bi-directional referral pathway between health systems and social service and lifestyle-change program providers to ensure patients with uncontrolled hypertension or hypercholesterolemia receive services based on their assessed needs. The Helping Hearts Program includes the following:

  1. CHI Referral Form Development.
  2. Electronic Bi-directional Referral Platform Establishment.
  3. CHW Training on CHI Referral and Services.
  4. CHI Service Delivery.
