Heart Health+ Counties 

The participating counties are as follows:

Each county selected a strategy to implement

Blood Pressure Self-Monitoring & Tobacco Cessation 

Blood Pressure self-monitoring is the regular measurement of blood pressure by the patient outside of the clinical setting, either at home or elsewhere. It requires the use of a home blood pressure measurement device by the patient to measure blood pressure at different points in time. This, along with clinical support, has been proven to improve access to care while making blood pressure control more convenient and accessible across the population.

The counties were instructed to implement a Blood Pressure Self-Monitoring Program in their community and partner with their local Tobacco Free Florida Quit Counselor to incorporate tobacco cessation messages in the program.

Hypertension Control Champions 

The Million Hearts® Hypertension Control Challenge is a competition to identify practices, clinicians and health systems that have worked with their patients to achieve hypertension control rates at or above 70% through innovations in health information technology and electronic health records, patient communication, and health care team approaches.

The counties were instructed to work with a health system towards achieving the goal of being recognized as a Hypertension Control Champion. They will implement standardized protocols for controlling high blood pressure in adults and for tobacco cessation to serve as clinical decision support at the point of care so no opportunities are missed to achieve control. They will also assist the health system with either reporting on the Million Hearts® clinical quality measures or implementing the Diagnosing the Undiagnosed toolkit.

100 Congregations for Million Hearts® 

The goal of 100 congregations is to enlist the support of faith based organizations to help raise awareness about risks for heart disease and stroke and educate individuals about prevention, including a focus on high blood pressure control. The counties were instructed to partner with at least 2 local faith-based organizations and assist them with joining 100 congregations for Million Hearts. They will assist with developing pulpit and other leadership messages about heart health and diabetes awareness. They will also work with the organizations to host two community events per organization which focus on heart disease prevention.

All strategies incorporate referring participants to Tobacco Free Florida’s Quit Your Way Programs, Diabetes Prevention Programs and Diabetes Self-Management Education Programs, as needed.

Heart Health+ pilot program launches in 17 counties
Heart disease has been the leading cause of death in Florida and the U.S. for over 70 years, which means it has affected a countless number of Floridians. Heart Health+ aims to prevent heart disease in the communities that need it the most. Launched November 2016, this pilot program implements evidence-based strategies to affect faith based organizations, health systems and local communities.

Congratulations to our HeartHealth+ counties
These counties were selected by analyzing heart disease, smoking, cholesterol, high blood pressure and other risk factors in each of Florida’s 67 Counties. The 17 counties selected showed the greatest need for heart disease prevention programs and were selected to participate in HeartHeart+.
