Low Protein Modified Foods:Children’s Medical Services (CMS) Plan will provide low protein modified foods from a non-Medicaid funding source for enrollees that qualify for such services due to Phenylketonuria (PKU) and other inherited metabolic diseases. Treating physicians can submit the request for low protein modified foods along with supporting documentation to the CMS Plan Central Office via fax at 850-921-524.
