Each year, the Immunization Section conducts surveys to evaluate immunization coverage levels among select age groups in Florida, and distributes the following reports.

Assessment of County Health Department (CHD) Clinic Immunization Coverage Levels

The Immunization Program completes the annual assessment of immunization coverage levels of 1- and 2-year-old children receiving services in County Health Departments to evaluate clinic immunization coverage rates. Survey of Immunization Levels in 2-Year-Old Children

This is an annual immunization survey of randomly selected 2-year-old children born in Florida. The survey provides estimated immunization coverage levels for certain counties, as well as statewide rates. This survey is conducted to determine immunization levels among 2-year-old children, to evaluate Florida's success in immunizing children on schedule, and to measure immunization levels of children based on high-risk maternal factors.

Child Care Facilities Immunization Survey

This assessment evaluates the immunization status of preschool children in randomly selected facilities in accordance with the recommended immunization schedules. Kindergarten and Seventh Grade Compulsory School Immunizations Annual Report and Survey

Each year, all public and private schools are required to submit reports in compliance with section 1003.22, Florida Statutes, and Florida Administrative Code Rule 64D-3.046, indicating the compulsory immunization status of their kindergarten and seventh grade students. Private schools must complete and submit an Immunization Annual Report of Compliance for Kindergarten and Seventh Grade (DH Form 684) to the local county health department (CHD) by October 1 each year. The results of all public and private school reporting are published as the Kindergarten and Seventh Grade Immunization Status Report.

Kindergarten and Seventh Grade Immunization Status Report

This report is an individual analyses for kindergarten and seventh grade status by county. Public and private schools submit reports regarding the immunization status of their kindergarten and seventh grade students, as required by Chapter 1003.22(8), Florida Statutes. Kindergarten and Seventh Grade Validation Assessment

This survey of a sample of student records in randomly selected schools is conducted to evaluate the accuracy of the annual Kindergarten and Seventh Grade Immunization Status Report results and to determine if any improvement in immunization levels occurred since the initial assessment. Immunization Assessments Ranking Report

This report is a compilation of the results from four previously published annual immunization assessments that provide county-specific data, into one convenient report. Those previously published assessments are 1) the Kindergarten and Seventh Grade Immunization Status Report, 2) Vaccine Accountability Report, 3) Assessment of County Health Department Immunization Coverage Levels in 2-Year-Old Children, and 4) the Survey of Immunization Levels in 2-Year-Old Children. Table 1 provides the data from each assessment in alphabetical order by county. Table 2 provides the data from each assessment in rank order by county, from the highest percentage achieved to the lowest. (Reports from some years are currently unavailable online.
