What is Partners in Care: Together for Kids (PIC: TFK)?

PIC: TFK is Florida’s Program of All-Inclusive Care for Children (also referred to as PACC). The program provides palliative care support services to enrollees of the Children’s Medical Services Health Plan. The PIC: TFK program consists of collaborative partnerships between state agencies, the managed care organization, and local hospice providers.

The PIC: TFK program is designed to enable children with a life-threatening illness, and their families, to access a support system that is continuous, compassionate, comprehensive, culturally sensitive and family centered from the point of diagnosis, with hope for a cure, through the provision of end-of-life care, if needed.

PIC: TFK services are provided by specially trained hospice providers. These providers have experience with palliative care and children.

 

What Services Does PIC: TFK Provide?

The PIC: TFK program helps meet physical, emotional, and spiritual needs of CMS Health Plan members. Services may include:

  • Support Counseling
  • Bereavement Support
  • Expressive Therapies
  • Pain and Symptom Management
  • Respite
  • Hospice Nursing Care
  • Personal Care
  • Volunteer Services

The Partners in Care: Together for Kids (PIC: TFK) program is a palliative care program that provides additional support services to children enrolled in the Children's Medical Services (CMS) Health Plan who have a life-limiting/threatening illness, as certified by their primary care or specialty physician.

ELIGIBILITY CRITERIA
Must be a CMS Health Plan member who:

  • Resides in a PIC: TFK service delivery area
  • Is diagnosed with a life-limiting/threatening condition
  • Resides in a community setting (e.g. home, group home)

How Do I Enroll?

Contact your CMS Health Plan Care Manager for more information and to see if your child qualifies for these services.

 

Maintain Your Child’s Services

To participate in the PIC: TFK program, the member must elect to receive at least two different PIC: TFK services within a three-month period. There is no limit on how long your child can be in the program. Each year, your child’s provider will be asked to confirm your child’s eligibility for enrollment in PIC: TFK. Your CMS Health Plan Care Manager will work with the provider when it is time to do so.

Thank you for your interest in the Partners in Care: Together for Kids (PIC: TFK) Program with Children’s Medical Services (CMS) Health Plan! Through the program, we seek to ensure children with life-threatening illnesses who are enrolled in the CMS Health Plan, and their families, are provided hospice-like support services through the PIC: TFK Program as described in section 409.912(11), Florida Statutes.

PIC: TFK Provider Requirements

  • Must be a licensed Hospice Provider in the State of Florida with an active Medicaid ID
  • Must be contracted and credentialed with Sunshine Health (our vendor) as a provider
