A commemorative certificate is signed by the current Governor and the state registrar of the Florida Department of Health’s Bureau of Vital Statistics.
The certificate is suitable for framing and preserving as a family heirloom. A commemorative certificate is not acceptable for official purposes.
Limited births were recorded between 1850 and 1917. Time of Birth was not collected on birth events between 1949-1969.

ELIGIBLE Requestors
Births less than 125 years old are confidential and may only be issued to:
- Registrant (name on the record) if of legal age (18)
- Parent(s) listed on the birth record
- Legal guardian (must provide guardianship papers)
- Legal representative of one of the above persons (must provide documentation)
- By court order (must provide copy of court order)
- Upon receipt of registrantโs death record, a commemorative birth certificate marked Deceased on the back may be issued to the spouse, child, grandchild, or sibling, if of legal age.
Photo ID Required
Valid photo identification is required when requesting a birth record. If the applicant is not an eligible requestor, the application must be accompanied with a notarizedย Affidavit to Release a Birth Certificateย signed by an eligible requestor, along with supporting documentation and a copy of valid photo ID of both the person authorizing release and the applicant.
Ordering a Certificate
Any person who willfully and knowingly provides any false information on a certificate, record or report required by Chapter 382, Florida Statutes, or on an application or affidavit, or who obtains confidential information from any vital record under false or fraudulent purposes, commits a felony of the third degree, punishable as provided in Chapter 775, Florida Statutes.
The Bureau of Vital Statistics offers walk-in or mail-in services. To save time, download and complete the forms you need. Same day service is not available for Commemorative Certificates.

Same day service is not available for commemorative certificates. Walk-in payments may be made in the form of cash, check, money order, Visa, and Mastercard. We do not accept contactless payments or tap to pay.

Normal processing time for computer-generated birth certificates is 3-5 business days. The commemorative certificate will follow in 4-6 weeks.
Request in writing
Please include the following information along with a copy of your identification, payment, and any supporting documentation. Information about the person named on the certificate:
- Full name on birth record
- Sex
- Date of birth (if unknown, specify range of years to be searched and provide additional fee of $2 per calendar year)
- City or county of birth
- Parent(s) full name(s) prior to first marriage

Photo Identification: Copy of applicantโs valid photo identification is required: driverโs license, state identification card, passport, military identification card.
Payment: Check or money order payable to Vital Statistics. DO NOT send cash. International payments must be made by cashierโs check or money order in U.S. Dollars from a U.S. bank.
- Florida Law imposes an additional service charge of $15 for dishonored checks.
- Fees are nonrefundable, except fee paid for additional copies when no record is found. These are refunded on written request.
