“Patient-Centered Transformation in Federally Qualified Health Centers in Florida”

Patient-centered practice transformation, such as the Patient Centered Medical Home (PCMH) model, has been identified as an important tool for improving the method of care delivery. Evidence shows that the PCMH model results in reduced costs and use of unnecessary services in primary care practices, however, little is known about its impact on outcomes for the nation’s Federally Qualified Health Centers (FQHC) population.
Dr. Harman will present results of a study that assessed the impact of PCMH transformation in FQHCs on clinical outcomes of patients with diabetes, and discuss a current proposal to evaluate implementation of the PCMH model among 48 FQHC clinic sites in Florida.
