The office of the Agency Clerk is responsible for accepting, filing and maintaining all legal documents received by the Florida Department of Health. This office responds to requests for copies of documents from the files of the clerk’s office; prepares, certifies and transmits the record on appeal when the Department acts as the lower tribunal; prepares Final Orders of the agency; and accepts service of process and subpoenas for the Florida Department of Health.

Single Audit Certification and Attestation

This portal is provided by DOH as an online fillable form to allow recipient and sub-recipient providers to certify annually to the Department whether: 

  1. the provider spent $1,000,000 or more in federal and/or $750,000 or more in state funds to conduct a single audit under the 2 CFR 200 subpart F or 215.97 F.S., and will submit a full single audit report no later than 9 months after the providerโ€™s fiscal year end date or 
  2. the provider spent less than $1,000,000 in federal expenditures and/or less than $750,000 in state expenditures. The provider will not be required to conduct or submit a single audit report (federal and/or state).
