In addition to overseeing Florida’s main immunization program, the Florida Department of Health provides information to assist with making informed decisions.

Obtaining Immunization Records

Immunization records are usually records that were provided to parents when the child received them and may also contain a medical record from the health care provider where the immunization(s) was given. There is no national database that maintains the immunization records for adults. If official copies of immunization records are needed, or if there is a desire to update personal records, the following places may provide some assistance to obtaining these records:

  • Ask parents/caregivers if they retained any records of immunizations (look in baby books or other saved documents from childhood).
  • Contact the high school and/or college attended for any records, if available.
  • Contact previous employers (including the military) that may have required immunizations for employment purposes.
  • Contact your health care provider or county health department. Records maintained by the health care providerโ€™s office may only be available for a limited number of years.
  • Contact your local county health department to determine if you have immunizations recorded inย Florida SHOTS.

If you have records in Florida SHOTS system, you can request your vaccination records directly from Florida SHOTS by completing anย authorization to disclose confidential informationย form. This form is for useย only by individuals aged 18 years and overย to request their own immunization history.ย 

Florida SHOTS is a free, statewide, centralized online immunization registry that helps parents, health care providers, and schools keep track of immunization records. Ask your health care provider for a personal identification number so you can view your childโ€™s immunization history and print a copy of your childโ€™sย Florida Certification of Immunizationย (DH Form 680).
