In addition to overseeing Floridaโ€™s main immunization program, the Florida Department of Health provides information to assist with making informed decisions.

Immunization Records

Immunization records are usually provided to parents when the child received them and may also contain other medical information from a health care provider. There is no national database that maintains the immunization records for adults. If official copies of immunization records are needed, or if there is a desire to update personal records, the following places may provide some assistance to obtaining these records:

  • Ask parents/caregivers if they retained any records of immunizations
  • Contact the high school and/or college attended for any records, if available
  • Contact previous employers (including the military)
  • Contact your local county health department to see if any records are on-file.
