Health improvement planning takes place at the state and local levels. To develop Florida’s State Health Improvement Plan, the Florida Department of Health led a diverse partnership of public health system partners to conduct a comprehensive state health assessment.
Each of Florida’s 67 county health departments participate in the health improvement planning process at the community level by facilitating a community health assessment and community health improvement plan.

This website provides data sets on environmental hazards and associated health outcomes. The purpose of these efforts is to inform communities about disease trends and to design interventions that lead to better health outcomes.
