Health care and laboratory providers are required to report conditions of public health importance to the Florida Department of Health as defined in Chapter 64D-3 of the Florida Administrative Code.
On this page, reporting requirements, methods for fulfilling them and associated resources are provided.
Health Care Providers
Requirement
Facilities providing health care services must submit case reports for reportable diseases and conditions, in line with the Health Care Practitioner Reporting Guidelines, to the appropiate county health department contact.
Medical records must be made available for epidemiologic investigations.
How to fulfill requirement

Fax the Practitioner Disease Reporting Form and medical records pertaining to the patient's infection to your county health department contact.
