The Telehealth Maternity Care Program offers prenatal and postpartum services and care for women throughout their pregnancy and up to one year after birth at no cost. Enrolled participants have access to comprehensive care and continued support from enrollment through the post-partum period.
Telehealth helps reduce barriers to care and compliments traditional in-person medical visits which are essential for performing certain tests and physical examinations.
Services
Screenings & Treatment
Screening and treatment services are provided for common pregnancy related complications, such as anxiety, cardiovascular conditions, diabetes, hypertensive disorders, infections, etc.
Referrals
Referrals can be provided along with connection to care and wrap-around services, such as nutrition counseling, stress management, housing placement, transportation, lactation support, etc.
Medical Devices
Remotely connected medical devices, such as scales and glucose monitors, may be provided.
Health & Wellness
Health literacy information and education is provided.
