In Florida, all allopathic and osteopathic physicians practicing pain-management by prescribing controlled substances to patients with chronic non-malignant pain are required to register and be inspected by the Florida Department of Health.
Pain management clinics are those that advertise in any medium for any type of pain-management services or where in any month a majority of patients are prescribed certain controlled substances for the treatment of chronic non-malignant pain. The Department will register and inspect the clinics to ensure the safety of the people in Florida.
If the pain management clinic will not be entirely physician-owned, registration with the Agency for Health Care Administration (AHCA) is required. The processing time for this registration can be lengthy. AHCA registration should be completed before applying for the pain management clinic registration with the Department.

- Print your confirmation page and keep on file for inspections
- Repeat this process for every clinic for which you are registered as the designated physician.

These documents request licensees share information that may be used to commit fraud against a licensee or patient. DOH is committed to protecting the integrity of DOH licensees and consumers. The DOH Office of Inspector General is in contact with the Agency for Health Care Administration and the Florida Department of Law Enforcement to investigate fraud perpetrated on our licensees.
Should you receive a document that appears to be from DOH, but you believe may be fraudulent, you may contact your local law enforcement agency or you may contact DOH at 850-488-0595 for instructions on how to proceed. Your cooperation and assistance is vital in protecting the continued integrity of DOH licensees and our consumers.
