Examination Scores
If you are a licensee and cannot find your State or Laws and Rules test scores via MQA Online Services and would like to have your scores mailed to another state, please fill out the score waiver and submit with your public records request in writing (fees may apply). For National test scores, please contact the National testing facility that administered your examination.
Disciplinary Actions
To determine the discipline history of a particular licensed health care practitioner, you may use the online Licensee Lookup System, which provides a simple “Yes” or “No” under the topic “Discipline on File” and the current status of the license. In addition, you can view disciplinary action taken after June 2006, use our Final Order and Emergency Action search engine.
Emergency suspension or restriction orders are not considered final agency action, but are imposed as specified by section 456.074, Florida Statutes, for serious violations relating to the commission of crimes, standard of care, or drug use, as well as for student loan defaults. The subject is entitled to a hearing before final action is taken by a regulatory board or by the Department.
Fees
- $.00 for 50 pages or less
- $.15 per page for the total number of pages if the number exceeds 50 pages (e.g., if the documents equal 52 pages, requestor is charged for 52 pages, not 2 pages)
- Microfiche – $.62 per page for the total number of pages if the number exceeds 15 pages (e.g., if documents equal 16 pages, requestor is charged for 16 pages, not 1 page)
- Certification of documents $25 each
- Research time is charged when in excess of 30 minutes and is based on the salary level of the lowest paid employee able to perform the research.
