Examination Scores

  • If you are a licensee and cannot find your state or laws and rules test scores through MQA Online Services and would like to have your scores mailed to another state, please complete the score waiver and submit with your public records request in writing (fees may apply).
  • For national test scores, please contact the national testing facility that administered your examination.

Disciplinary Actions

To determine the discipline history of a particular licensed health care practitioner, search the Licensee Lookup System, which provides a simple “Yes” or “No” under the topic “Discipline on File” and the current status of the license. You can also view disciplinary action taken after June 2006, use search under final order and emergency action.

Emergency suspension or restriction orders are not considered final agency action, but are imposed as specified by section 456.074, Florida Statutes, for serious violations relating to the commission of crimes, standard of care, or drug use, as well as for student loan defaults. The subject is entitled to a hearing before final action is taken by a regulatory board or by the Department.

Fees

  • 50 pages or Less: $0
  • 51 pages or More: $0.15 per page for the total number of pages (e.g., if the documents equal 52 pages, requestor is charged for 52 pages, not 2 pages)
  • Microfiche: $0.62 per page if exceeds 15 pages (e.g., if documents equal 16 pages, requestor is charged for 16 pages, not 1 page)
  • Certification of Documents: $25 each

Research time is charged when in excess of 30 minutes and is based on the salary level of the lowest paid employee able to perform the research.
