Through the WebLIMS Portal, authorized submitters and epidemiologists may access the BPHL LabWare system to submit orders and to view and print reports for their patients or patients in their authorized territory/region.

User Access
To be granted access to the WebLIMS Portal, complete the following two forms:
If you would like your results to be faxed to you, complete the additional Secure Auto Fax Form.
Submit the forms to DLBPHLLAR@FLHealth.gov.
Please allow 3 weeks for processing. Any cancellations of access forms must be done in writing by fax (904-791-1567) or email address above. If a user’s account is inactive for 15 days, the account will be deleted and the entire application process must be repeated to regain access.
