License Types and Fees
Full License
The requirements for licensure can be found in sectionย 483.914, Florida Statutes, and include:ย
- Is of good moral character
- A masterโs degree from a genetic counseling training program or its equivalent as determined by the Accreditation Council of Genetic Counseling or its successor or an equivalent entity; or
- A doctoral degree from a medical genetics training program accredited by the American Board of Medical Genetics and Genomics or the Canadian College of Medical Geneticists
- Certification asย aย genetic counselor by the American Board of Genetic Counseling, Inc., the American Board of Medical Genetics and Genomics, or the Canadian Association ofย Genetic Counsellors; orย aย medical or clinical geneticist by the American Board of Medical Genetics and Genomics or the Canadian College of Medical Geneticists.ย
Temporary License
The requirements forย temporaryย licensureย can be found in sectionย 483.914, Florida Statutes. The Department may issue a temporary license for up to two yearsย for an applicantย who has notย completed the certification examination requirement but is eligible to sitย for the examination,ย andย who meets theย following requirements:ย
- Is of good moral character.
- A masterโs degree from a genetic counseling training program or its equivalent as determined by the Accreditation Council of Genetic Counseling or its successor or an equivalent entity; orย
- A doctoral degree from a medical genetics training program accredited by the American Board of Medical Genetics and Genomics or the Canadian College of Medical Geneticists
Out of State Telehealth Registration
The out-of-state telehealth provider registration is for health care practitioners licensed outside of Florida ONLY. Florida licensees can already provide telehealth services to patients in Florida that they can treat in person.
Health care practitioners with an out-of-state license or certification that falls under section 456.47(1)(b), F.S, qualify for an out-of-state telehealth provider registration number when they meet the following requirements:
- Submit the completed Application for Out-of-State Telehealth Provider Registration;
- Maintain an active, unencumbered license from another state, the District of Columbia, or a possession or territory of the United States (license verification is required);
- Not have a pending investigation, discipline, or revocation on your license within the last five years;
- Designate a duly appointed registered agent for service of process in Florida (see Application for Out-of-State Telehealth Provider Registration)
- Maintain liability coverage or financial responsibility for telehealth services provided to patients in Florida in an amount equal to or greater than Florida health care practitioner requirements
- Not open a Florida office or provide in-person health care services to Florida patients
- Only use a Florida-licensed pharmacy, registered nonresident pharmacy, or outsourcing facility to dispense medicinal drugs to patients in Florida (pharmacists only)
Renewals
Your license will expire at midnight, Eastern Time,ย December 31, 2023. At least 90 days before your license expires, the Department will mail you a renewal notification postcard to your last known address of record. You will have until midnight on the day your license expires to renew.
Failure to renew an active or inactive license by the expiration date will result in the license being placed in delinquent status. Failure by a delinquent licensee to renew before the expiration of the current licensure cycle renders the license null and void without any further action by the board or the Department.

Renewal Instructions:
- Visitย FLHealthSource
- Enter your Personal Identifying Information – if screen displays failed, select other login option and enter your existing User ID and password to access your account.
- Confirm or update your MQA Services Account email address
- You are now in the dashboard. During renewals, aย โRenew My Licenseโย option will populate no later than 90 days prior to your license expiration date. If you do not see theย โRenew My Licenseโย option, please check back at a later time.
Complete a mandatory background screening or retain* your current fingerprints. To learn more, please visitย FLHealthSource.gov/Background-Screening.
*Required by section 456.0135(6)(1), Florida Statutes, a $43.25 fee is required for FDLE to retain your fingerprint for background screening during this licensure cycle. This fee is collected at the time of license renewal if your fingerprint retention is expiring this licensure period. If your fingerprint retention expires in the next 60 days, you will need to pay separately through theย Clearinghouse Applicant Initiated System.
A complete application and fee are required to be submitted online or postmarked by mail no later than midnight on the expiration date to avoid your license from becoming delinquent or null and void.

Special Circumstances
Applicants with Health History
Applicants who answer โYesโ to any of the Health History questions on the application are required to submit the following documentation to the board office:ย
- Self-Explanationย โ Applicants must submit a letter in your own words explaining the medical condition(s) or occurrence(s). Include a description of all treatments and diagnoses you have received for any condition(s)/impairment(s) you are/have been treated for. Include all medications prescribed and all physicians/counselors that have provided treatment.ย
- Physician(s) Letterย โ Applicants must submit a statement from your treating physician(s)/counselor(s) for each condition you are/were being treated for andย whether or notย you are currently able to safely practice genetic counseling. The physicianโs statement should include all DSM IIIR/ DSM IV, Axis I, II, and III diagnoses.ย
Applicants with License Discipline History
Applicants with prior disciplinary actions on any license from any state are required to submit the following supplemental documentation:ย
- Board Actionsย โ Certified copies of documents relative to any disciplinary action taken against any license. The documents must be certified and submitted by the agency or board that finalized the disciplinary action.ย
- Self-Explanationย โ Applicants who have listed license discipline on the application must submit a letter to the board personally describing the circumstances that led to the disciplinary action. The letter should also include a thorough description of the rehabilitation taken by the applicant since the time of the event which would facilitate prevention of future occurrences.ย
Applicants with Criminal History
Applicants who have been convicted of a misdemeanor or felony; or entered a plea of guilty, nolo contendere, or no contest to any crime in any jurisdiction; even if adjudication was withheld by the court; must answer the criminal history questions on the application and provide the following supplemental documentation:ย
- Final Dispositions/Arrest Recordsย โ Final disposition records for offenses can be obtained at the clerk of the court in the arresting jurisdiction. If the records are not available, a letter on court letterhead must be sent from the clerk of the court attesting to records unavailability.ย
- Completion of Sanctionsย โ Probation and financial sanction records for offenses can be obtained at the clerk of the court in the arresting jurisdiction. Parole records for offenses can be obtained from the Department of Corrections or at the clerk of the court in the arresting jurisdiction. If the records are not available, a letter on court letterhead must be sent from the clerk of the court attesting to records unavailability.ย
- Self-Explanationย โ Applicants who have listed offenses on the application must submit a letter personally describing the circumstances of the offense. The letter must include the date of the original offense, the charge, and the jurisdiction where the incident occurred.ย
All applications with โYesโ responses to the history questions on the application will be reviewed by the board office for possible consideration by the Board of Orthotists and Prosthetists. If required, the board staff will notify you of the date that your application will be presentedย in the event thatย you would like to attend.ย
Electronic Fingerprinting
The applicantย who is applyingย for licensureย as aย member ofย theย armedย forcesย or their spousesย in good standing with administrative boards or the Department,ย pursuantย to section 456.024, F.S., must complete a criminal background screening by submitting fingerprints electronically through a service provider approved by the Florida Department of Law Enforcement. The results will be returned to the Care Provider Background Screening Clearinghouse and are made available to the Department for review during the licensure process.
Applicant fingerprints will be retained by FDLE and the clearinghouse. All costs for conducting criminal history background screening and the retention of fingerprints are the responsibility of the applicant.ย
It is important to use the correct Originating Agency Identification (ORI) when submitting fingerprints. If an incorrect ORI number is provided to theย Livescanย service provider, the Department will not receive your fingerprint results.ย
The ORI number forย Genetic Counselingย is EDOH2017Z.ย
The applicant is responsible for selecting an approvedย Livescanย service provider and for ensuring the results are reported to the Department. Print the electronic fingerprinting form, complete the information, and take the form to aย Livescanย service provider.
Upon completion of electronic fingerprinting, document the Transaction Control Number. This number is useful to track the fingerprint records in the event they are not properly transmitted to the Department.ย

Health Care Fraud and Disqualification for License, Certificate or Registration
Effective July 1, 2012, Section 456.0635, Florida Statutes (F.S.), provides that health care boards or the department shall refuse to issue a license, certificate or registration and shall refuse to admit a candidate for examination if the applicant:ย
- Has been convicted of, or entered a plea of guilty or nolo contendere to, regardless of adjudication, a felony under Chapter 409, F.S., (relating to social and economic assistance), Chapter 817, F.S., (relating to fraudulent practices), Chapter 893, F.S., (relating to drug abuse prevention and control) or a similar felony offense(s) in another state or jurisdiction unless the candidate or applicant has successfully completed a drug court program for that felony and provides proof that the plea has been withdrawn or the charges have been dismissed. Any such conviction or plea shall exclude the applicant or candidate from licensure, examination, certification, or registration, unless the sentence and any subsequent period of probation for such conviction or plea ended:ย
- For the felonies of the first or second degree, more than 15 years from the date of the plea, sentence and completion of any subsequentย probation;ย
- For the felonies of the third degree, more than 10 years from the date of the plea, sentence and completion of any subsequentย probation;ย
- For the felonies of the third degree under section 893.13(6)(a), F.S., more than five years from the date of the plea, sentence and completion of any subsequentย probation;ย
- Has been convicted of, or entered a plea of guilty or nolo contendere to, regardless of adjudication, a felony under 21 U.S.C. ss. 801-970 (relating to controlled substances) or 42 U.S.C. ss. 1395-1396 (relating to public health, welfare, Medicare and Medicaid issues), unless the sentence and any subsequent period of probation for such conviction or pleas ended more than 15 years prior to the date of the application;ย
- Has been terminated for cause from the Florida Medicaid program pursuant to section 409.913, F.S., unless the candidate or applicant has been in good standing with the Florida Medicaid program for the most recent fiveย years;ย
- Has been terminated for cause, pursuant to the appeals procedures established by the state or Federal Government, from any other state Medicaid program, unless the candidate or applicant has been in good standing with a state Medicaid program for the most recent five years and the termination occurred at least 20 years before the date of the application;ย
- Is currently listed on the U.S. Department of Health and Human Services Office of Inspector Generalโs List of Excluded Individuals and Entities.
Continuing Education
Continuing education requirements will be consistent with nationally accepted standards of the American Board of Genetic Counseling, Inc.
The Department will renew your license upon:
- Receipt of a complete renewal application along with a $5 unlicensed activity fee
- Uploading a copy of an active certification from the American Board of Genetic Counseling, Inc.(ABGC) to CEBroker
- Uploading proof of completion of a 2-hour prevention of medical errors course
