Licensing requirements
A Florida license is required for anyone in the state who tattoo the body of another human; individuals who practice cosmetic tattooing, permanent makeup, microblading, or micropigmentation; and students or apprentices who tattoo other humans as part of their tattoo training.
Licensing services are available through the local county health department, in the county that you live.
Initial License Requirements
Submit your completed application to the local county health department, in the county that you live. Before applying for the tattoo artist license, please contact your local county health department for information about fees and mailing address.
- Completed Application for Tattoo Artist License and $60 application fee (additional fees may vary by location)
Application must include the name of the licensed tattoo establishment where the artist will perform or intends to perform tattooing. - At least 18 years of age
- Copy of government-issued photo ID
- Proof of completion of bloodborne pathogen and communicable diseases course for tattoo artist (minimum of 70% score)
Tattoo Artist License Renewals
Submit the following to the local county health department where you reside:
- Completed Application for Tattoo Artist License, select renewal.
Application must be completed with the name of the licensed tattoo establishment where the tattoo artist will perform tattooing or intends to perform tattooing. - $60 renewal fee (additional fees may vary by location)
Reactivate an Expired Tattoo Artist License
Submit the following to the local county health department where you reside:
- Completed Application for Tattoo Artist License and $85 fee ($60 renewal, $25 reactivation) must be issued by the county where artist lives, regardless of area of work.
Guest Tattoo Artist Registration

A guest tattoo artist registration is required for any tattoo artist who plans to tattoo in Florida and who also holds an active tattoo artist license, registration, or certification issued by a jurisdiction outside of Florida.
If your state does not require a tattoo artist license registration or certification, you must apply for a Florida tattoo artist license.
Guest tattoo registration requires the following:
- Completed Application for Guest Tattoo Artist Registration, $35 fee (additional fees may vary by location)
Application must contain the name of the licensed tattoo establishment where the tattoo artist will perform tattooing or intends to perform tattooing - At least 18 years of age
- Copy of government-issued photo ID
- Proof of active license, registration, or certification outside of Florida
- Documentation of completed tattooing bloodborne pathogen and communicable diseases course (at least 70% score) unless license is issued by a jurisdiction recognized as having educational requirements which meet or exceeds the requirements of section 381.00775, F.S.
Guest registration is valid up to 14 consecutive days in Florida.
