The Florida Department of Health’s Division of Children’s Medical Services’ Specialty Networks for Access and Quality (SNAQ) Program supports children and youth with special health care needs by providing access to quality health care services, including the transition of health care.
Through its collaborative partnerships with various organizations, the SNAQ Program promotes comprehensive, coordinated, family-centered, and evidence-based systems of care for children with the following conditions:
Chronic Kidney/Renal Disease
Provides services to infants and children from birth to 21 years of age with less than 75% normal kidney function for a period of six weeks or more (chronic renal disease) who are enrolled in a participating provider’s comprehensive children’s kidney failure center program(s).
Craniofacial & Cleft Lip / Cleft Palate
Provides services to infants and children from birth to 21 years of age with craniofacial abnormalities or cleft lip or cleft palate disorders who are enrolled in a participating provider’s pediatric craniofacial program.
Endocrine and Diabetes
Provides services to infants and children from birth to 21 years of age with endocrine disorders, including but not limited to diabetes, who are enrolled in a participating provider’s pediatric endocrinology program.
Hematology and Oncology
Provides services to infants and children from birth to 21 years of age with hematologic or oncologic disorders who are enrolled in a participating provider’s pediatric hematology/oncology care center program(s).
HIV
Provides services to infants and children from birth to 21 years of age who are exposed to, at risk for, or infected with HIV, who are enrolled in a participating provider’s pediatric HIV program.
Pulmonary Disorders
Provides services to infants and children from birth to 21 years of age with pulmonary disorders who are enrolled in a participating provider’s pediatric pulmonary program.
To achieve its goals and objectives, the SNAQ Program partners with universities and hospitals possessing the necessary expertise, infrastructure, and network to serve the pediatric populations. Our current SNAQ Program partners include:
