The Heart Health Plus Program was created with the purpose of reducing the burden of heart disease and related risk factors in communities with a high prevalence of cardiovascular disease, hypertension (high blood pressure), and high cholesterol.

County health departments have implemented Heart Health Plus activities, including blood pressure self-monitoring programs, within their communities. County staff work with community partners to provide screening and outreach. They collaborate with health systems to establish policies and systems that encourage the use of prevention and management programs. They also work to increase referrals to additional self-management and healthy behavior support services:Â
